Position Summary:

The Admissions Nurse plays a critical role in coordinating and overseeing the admission process for new resides entering the skilled nursing facility. This nurse ensures a smooth, timely, and compassionate transition for residents and their families while verifying clinical appropriateness, gathering accurate health data, and working collaboratively with interdisciplinary teams.

Key Responsibilities

• Admission Coordination

• Receive and review referrals from hospitals, physicians, case managers, and other sources.

• Conduct clinical pre-admission screenings to access potential residents' needs and ensure appropriate placement

• Coordinate with the IDT team members including (nursing, therapy, social services, dietary, etc.) to prepare for the resident's arrival.

Qualifications

Education: Graduate of an accredited nursing program.

Licensure/Certification:

Current RN license in the state of practice

Skills:

• Strong clinical assessment and communication skills

• Ability to multitask and work in a fast-paced environment

• Knowledge of state and federal regulations

• Experience with electronic medical records (PCC) systems preferred.

The Environmental Services Director ensures that the Healthcare Center is a safe, sanitary, and organized environment. He or she is responsible for managing the housekeeping and laundry departments and ensuring proper staffing levels are maintained. Experience and/or Pre-requisites:

What You'll Do
